5) Caste Certificate for SC, ST, OBC, Baxi (if applicable)
6) Self attested Photocopy of Aadhar card of student and parents
7) Photocopy of Blood group report
8) Photocopy of Immunization record
9) Original School leaving certificate (1st grade onwards)
